Registration Forms and Medical Information
We ask everyone who attends a class or one-to-one session to fill in a Registration form. This asks you for name, address, email address and phone-numbers and has a large health information section.
To be able to treat or teach you effectively, we need to ask you information about health conditions and injuries. It is very important you give accurate and current information to us, and keep us up-to-date with your state of health.
Registration forms are stored online in an encrypted and password protected cloud. Only staff and teachers working on behalf of Spot on Fitness Limited have access to your information.
Any personal information that is kept electronically is stored securely and is password protected. It can be deleted on your instruction or when you cease to be a client of Spot on Fitness Limited.
Spot on Fitness Ltd is registered with the ICO registration number: ZB271805.
Your details will be kept on file for three years from your last session, in accordance with the Statute of Limitations Act 1980. After this time they will destroyed and deleted.
